The California Massage Therapy Council has now uploaded the application for state certification. If you are going to apply for state certification, the CAMTC requires an official transcript be sent directly from your school. They also require the school's correct address.
The current address for Somatherapy Institute is 70-225 Highway 111 Suite B&C, Rancho Mirage, CA 92270.
The price for a transcript is $30.00 for one signed official transcript, mailed directly to the CAMTC, or one mailed directly to the student, or any other licensing agency, or school.

PLEASE NOTE:  The form below is for your official name, address and birthdate which will help us find your transcript and create a new official transcript to be sent to the CAMTC.  (We know the address of the CAMTC and where to send the official transcript. Please use the form for your information not the CAMTC address.)  If you attended school under a different name please let us know.
Please be aware that the CAMTC requires at least 100 hours devoted to instruction addressing anatomy and physiology, contraindications, health and hygiene, and business and ethics. If you do not  have 100 hours of A&P (that includes kinesiology), contraindications, health and hygiene, business and ethics, you will need to take those classes to meet state requirements.
