Transcript Request

The California Massage Therapy Council has now uploaded the application for state certification. If you are going to apply for state certification, the CAMTC requires an official transcript be sent directly from your school. They also require the school's correct address. The current address for Somatherapy Institute is: 70-225 Highway 111 Suite B&C, Rancho Mirage, CA 92270. The price per official transcript is $45.00 for one signed, embossed official transcript, mailed directly to CAMTC, or one mailed directly to the student, or any other licensing agency, or school (please allow 3-4 weeks for delivery). An unofficial transcript (PDF emailed to you for your personal records) is $20.00.

PLEASE NOTE: The form below is for your official name, address and birthdate which will help us find your transcript and create a new official transcript (if needed) to be sent to CAMTC. (We know the address of CAMTC and where to send the official transcript. Please use the form for your information, not the CAMTC address.) If you attended school under a different name please let us know.

Please acknowledge that CAMTC requires a minimum of 100 hours devoted to instruction addressing anatomy and physiology, contraindications, health and hygiene, and business and ethics. In addition to 100 hours; 35 hours of contraindications are required as well as 250 hours of hands-on training. The rest of the hours are filled in with other classes that meet requirements and approval. You MUST meet all minimal hour requirements. We are here to help you through this.
